Title: Dementia Adviser

At Alzheimer’s Society, we believe no one should face dementia alone. Every conversation, every visit and every small moment of reassurance can make a real difference. We are looking for a warm, thoughtful and organised Dementia Adviser to join our supportive team in North Wales working 35 hours per week on a fixed term contract until March 2027.
We work with 3rd sector partners as part of the North Wales Memory Support Pathway - You must be able to travel independently across all areas of North Wales, but specifically Central and East areas to complete visits and attend meetings.
You will work closely with people living with dementia and those who care about them, providing ongoing support. You will listen carefully, understand what matters most to each person and offer guidance that helps them feel steadier and more confident as their situation changes.
Your days will be varied and meaningful. You might be visiting someone at home, offering calm reassurance over the phone, or working closely with other professionals to make sure the right support is in place. We operate a triage system for new referrals and you will play your part in making sure people are responded to quickly and thoughtfully. You will manage your own caseload, keeping clear and accurate records, while always seeing the person behind the paperwork.
You will be part of a caring and dynamic local team who share ideas, reflect together and support one another. There will be time to meet as a team and opportunities to keep growing your knowledge of dementia, with your manager and colleagues alongside you.

About the role
This role is responsible for the leadership, performance, and integration of the Caseworker team and the Practical Team administrative function. The postholder ensures that both functions operate as a coordinated service, with effective systems, processes, and workflows in place and consistently adhered to. The role ensures high-quality, timely, and person-centred service delivery, enabling clients to remain safe, independent, and supported in their own homes. High quality advice and guidance are key to this role and adhering to the Advice Quality Standard framework (AQS).
Currently the team is 14 members of staff, but this may increase subject to operational demand and new business development opportunities.

This role will work on our 12-month WeGrow programmes in Flintshire, supporting our Gappies with work placements, life coaching, new skills, experiences, and a further 6-months of dedicated support.
You will be the ultimate role model for our Gappies, showing them what a responsible employee looks like, sharing life skills and experience in a way which encourages them to choose positive behaviours.
Our WeGrow programme has been the flagship programme for WeMindTheGap for over a decade and includes:
- Launchpad – a coaching course to answer the questions ‘Who am I? What do I want? How am I going to get it?’
- Amazing Experiences – horizon broadening activities from days out exploring the local area, health and nutrition workshops, an outdoor adventure residential trip, and even tea with the Mayor.
- Essential Skill Development – the chance for Gappies to fill any gaps they may have, whether this is working towards improving their Maths and English, learning about nutrition and how to cook healthy, nutritionally balanced meals, or improving their digital literacy.
- Six-months of paid work – providing Gappies with four placements within different sectors offered by our employer partners.
It will be your responsibility to help lead Gappies through this journey with unashamed love, care and support. Every journey will be different, and it will take all of your experience, professionalism and patience to ensure each Gappie has the best programme they can.

We are looking to recruit a talented, dynamic and professional Engagement Manager to join our team. In this role, you will lead all aspects of the Flintshire Befrienders project.
Flintshire Befrienders is funded by the National Lottery Community Fund and supports older people experiencing social isolation and loneliness. Our team of Volunteer Befrienders provide companionship, help to build confidence, and encourage engagement in the local community. The project consists of:
- One-to-one befriending in a person’s home, or over the telephone
- Community buddying – support to attend local community groups and events
- Volunteer-led events – activities that older people can engage in, based on their interests
The right candidate would be someone with experience of supporting and managing volunteers, working with older people facing isolation and loneliness, as well as someone who can work flexibly and demonstrate a commitment to our organisational values.

Also to contribute to the promotion of housing policy initiatives and best practice and to work in a constructive and collaborative way with other services, including local authorities and housing associations when it is in the best interest of our users.

The Early Intervention Project delivers itself or provides direct links to a whole range of interventions that prevent the risk of homelessness. These include housing, financial inclusion and welfare benefits information, general guidance and support, employment, training and education services and health, well-being and social care advice amongst others. Shelter Cymru Housing Intervention Workers (HIWs) are key to this approach as the first point of contact and support.
This is a busy and reactive service, and each day is different. At first contact such as at a face-to-face surgery venue, or on receipt of referrals and a follow up phone call, the HIWs will act as a triage, gathering information to either provide assistance themselves or allocate the case to the appropriate partner.
A holistic (whole person) assessment identifying all factors that may be contributing to a risk of homelessness for the individual will be undertaken. This aims to capture all the issues that may be present in the person’s life. In doing this, the project will listen and seek to understand what matters to the person, the problem that they are presenting with (for example, rent arrears) as well as the other areas of their life that may be impacting on this (such as loss of employment or relationship difficulties).
The project workers will then work together with the individual to help to identify ways these may be addressed and mitigated. Where needs can be met by the project’s Delivery Partners this will commence rapidly. Where there are wider needs such as mental health/well-being, employment or relationships, the person can be linked into the wider project network.
We know that a lot of homelessness in Denbighshire is due to tenancies ending in the private rented sector (PRS). This role will also provide the operational contact for any landlords accessing the My Home Denbighshire service. As well as providing practical information and signposting for landlords/agents in relation to a property, the post will also provide housing intervention support to PRS tenants with the objective of preventing a situation escalating into homelessness. Where there is no prospect of saving the tenancy, the project team will work with tenants to seek alternatives to avoid homelessness.
The use of the Elemental system will mean that the main delivery partners are completely connected and all using the same referral, case management and reporting system.
